The deadline for the application is January 15, 2009.
Please send and electronic version of the completed application via email to RFA@pkdcure.org. Mail the original, signed application with all attachments to Amy Freaney at the PKD Foundation by midnight on January 15, 2009. Late applications post-marked after January 15, 2009, will not be accepted. The receipt deadline will not be waived.
