Application Guidelines

 

 

1. All applications must be submitted electronically by the indicated deadlines. Files must be in Adobe Acrobat format (version 5.0 or higher) and no larger than 1.5 MB in size. Please visit our web site www.pkdcure.org for further application information. Applications must be typed, single-spaced, 12-point font with at least one-inch margins. Please send application in electronic form to RFA@pkdcure.org

2. Complete and sign the enclosed "2008 Translational Research Grant-in-aid Application Cover Page" (DOC) and attach to the front cover of the written copy you will submit. The original, signed version of the complete application will be considered the primary document. It must be mailed to the PKD Foundation and post-marked no later than the indicated deadlines.

3. Please submit the materials listed below, in the order they are listed: (30 page suggested maximum limit)

a. Proposal summary and its relevance to PKD research (one page limit). Describe project in a scientific abstract understandable to a multidisciplinary group of scientific reviewers.

b. Separate one paragraph lay summary of 100-150 words (one page limit).

c. Specific aims and rationale (two page limit).

d. Background information including significance to clinical medicine (three page limit).

e. Research plan and Methods of Study (seven page limit). Include a brief description of laboratory space, equipment, clinical facilities access to patient populations and other resources necessary to the project.

f. References (two page limit). Please format in 12 pt font: Authors, (year), title, journal, volume, pages.

g. Budget (DOC) form must be in US $ - (one page limit). Must include the following, must use this form:

i. Personnel: Salaries, Wages & Fringes: No more than 50% of Direct Costs ($45,454) may be used for salary and fringes. Justify any proposed use of the award for salary support for the applicant.

ii. Consultant Costs (Maximum of $10,000)

iii. Equipment (Maximum of $12,000)

iv. Supplies

v. Travel

vi. Animal Care

vii. Other Costs - including patient care

viii. Indirect Costs (Maximum of 10% of Direct Costs. DC + ID = $100,000)

h. Biosketch (DOC) formMust use this form. Please follow example on form. (two page limit per Principal and Co-Investigators)

i. Other Support (DOC) formMust use this form. Please follow example on form. (two page limit per Principal or Co-Investigator)

j. Signed single page IRB approval sheet and a summary of the clinical protocol, if applicable. (two page limit)

k. Signed IACUC approved animal use protocol if applicable.

m. All re-submission grants must have reviewer’s comments addressed in proposal. Highlight where reviewer’s comments have been addressed and prepare a cover letter to the Review Committee noting the highlights. Cover letter will not count towards total page count.

n. Checklist must be included with application.
