Tolvaptan binds to the vasopressin receptor on renal cells and blocks cAMP (an intracellular second messenger) formation inside the cell.  Fluid secretion and cell proliferation both reduced resulting in cyst and kidney size.  This drug is not effective in slowing liver cyst formation because liver cells do not have vasopressin receptors.
