Track your Walk for PKD Donations Keep this form for your records to track of all of the donations you collect during your Walk for PKD campaign.

Donation Submission Form Use this form if you are a registered walker to send collected donations.

Cash Donation Form Use these receipts for any cash donations you receive from family and friends. Print off as many as you need, then collect the cash and convert it to a cashier’s check to submit it to the PKD Foundation.
