The deadline for applications is August 15, 2008.

Please send an electronic version of the completed application via email to RFA@pkdcure.org. Mail the original, signed application with all attachments to Amy Freaney at the PKD Foundation by midnight on August 15, 2008. Late applications post-marked after August 15, 2008, will not be accepted. The receipt deadline will not be waived.
