Application Guidelines
1. All applications must be submitted electronically. Application and attachments must also be mailed to the PKD Foundation. Files must be in Adobe Acrobat format (v. 5.0 or higher). It must be typed, single-spaced in 12 point Times New Roman font.
2. Complete and sign the Bridge Grant-in-Aid Application Cover Page (DOC). Attach it to the front cover of the written document you submit. The original, signed version of the complete application will be considered the primary document and must be post-marked no later than the deadline date.
3. Please submit the materials listed below in the order listed.
a. PKD Bridge Grant Application Cover Page
b. NIH summary sheet
c. Complete NIH RO1 grant proposal including appendices
d. Proposed budget for one year.
