Application Guidelines

 

1. All applications must be submitted electronically. Files must be in Adobe Acrobat format (version 5.0 or higher) and no larger than 1.5 MB in size.  Applications must be typed, single-spaced, 12-point font with at least one-inch margins.

 

2. Complete and sign the "Research Grant-in-aid Application Cover Page (DOC)" and attach to the front cover of the written copy you will submit.  The original, signed version of the complete application will be considered the primary document.  It must be mailed to the PKD Foundation and post-marked no later than August 15, 2008.

 

3. Please submit the materials listed below, in the order they are listed:  (20 page maximum limit; if application exceeds the maximum number of pages allotted for each section, it will be returned without review):
      a) Proposal summary and its relevance to PKD research (one page limit).
      b) Separate one paragraph lay summary of 100-150 words (count as one page)
      c) Specific aims and rationale (one page limit).
      d) Background information (three page limit).
      e) Research plan and Methods of study (six page limit). Preliminary data may be included.
      f) References (two page limit). Include article titles, 12 pt. font.
     g) Budget (DOC) form must be in US $ - (one page limit; if request is for more than 1 year, please provide a column for the second year’s request).  Must use form. Must include the following:
          i.  Personnel: Salaries, Wages & Fringes, Consultant Costs (P.I. salary only maximum of $32,500)
          ii.  Equipment (Maximum of $12,000)
          iii.  Supplies
          iv.  Animal Care
          v.  Travel
          vi.  Other Expenses        

          vii.  Indirect Costs (Maximum of 10% of grant request, i.e., $7,727 for a $85,000 award)
     h) Biosketch (DOC) form (two page limit). Must use form. Please follow example on form.
     i) Other Support (DOC) form (one page limit). Must use form. Any overlap of financial support must be fully explained.

 

4. All graphs, tables and charts should be prepared in black ink.   Photographs, oversized documents or materials in the body of the application that do not reproduce well should be inserted in original form in each copy of the application. 

 

5. Number pages sequentially at the bottom center, starting with the Proposal Summary

 

6. Label each page with applicant’s last name and first name initial in the upper right corner.

 

7. The original application should not be bound or stapled but secured with a rubber band, binder clip, or paper clip.

 

8. Please include a copy of applicant's animal protocol approval letter/form or human subjects certification, if applicable.  It is not necessary to include the actual protocol.

10. All re-submission grants must have reviewer's comments addressed in proposal.
